Hydronic heaters (wood boilers) explained: indoor, outdoor, and gasification

Short answer: a hydronic heater (wood boiler) burns wood or pellets to heat water instead of air, then distributes that heat through radiators, radiant floor loops, a heat exchanger in a furnace plenum, or domestic hot water — letting one appliance, often located outside the living space, heat an entire house.
How hydronic distribution differs from a room heater
A wood stove or forced-air furnace heats air directly and moves it by convection or ductwork. A hydronic heater instead heats a water loop, which carries thermal energy far more efficiently over distance than air does. That water can then feed baseboard radiators, panel radiators, radiant floor tubing, a coil in an existing forced-air furnace's plenum, or an indirect domestic hot water tank — often several of these at once from one firebox. Because the appliance itself can sit outside or in an outbuilding, hydronic heaters also solve the "wood smell and mess indoors" objection that keeps some households away from wood heat entirely.
Indoor, outdoor, and gasification designs
Outdoor wood boilers sit in a small shed-like structure outside the house and send insulated supply and return lines underground to the building. Indoor boilers install inside a basement or mechanical room like a conventional oil or gas boiler, venting through a chimney. Gasification boilers — available in both indoor and outdoor forms — burn wood in two stages: an initial gasification chamber breaks the wood down and drives off combustible gases, which then burn at very high temperature in a secondary chamber, extracting far more heat and burning far cleaner than a simple firebox. Many gasification designs pair with a thermal storage tank: the boiler runs hot and clean at full output to charge the tank, then the house draws heat from storage as needed, avoiding the smoldering idle that produces the most smoke and creosote.
Why older outdoor boilers got a bad reputation — and got banned in places
Early outdoor wood boilers, especially pre-certification models from the 1990s and 2000s, were frequently run at a low smolder to avoid overheating the water loop, which is exactly the condition that produces the most visible smoke and highest particulate emissions. Complaints about neighboring smoke led a number of states and municipalities to restrict or ban outdoor wood boilers outright, or to impose stack-height and setback rules well beyond ordinary wood stove requirements. Some of those older local restrictions predate current EPA certification and may not distinguish a certified low-emission unit from an uncertified one — always check your specific state and local rules before installing a hydronic heater, particularly an outdoor unit, regardless of what the appliance is certified to nationally.
What certification changed

Is a hydronic heater the right choice?
Hydronic heaters make the most sense when you want whole-house (and possibly domestic hot water) heat from one wood or pellet fire, especially in a home already built around baseboard, radiator, or radiant-floor distribution rather than ductwork. If a single room or open floor plan is all you're heating, a room heater — wood or pellet stove is simpler and cheaper to install; if you need ducted whole-house air distribution instead of water, a wood forced-air furnace is the more direct fit.
